problems falling asleep

John 31 March 2006 20:56:11
 Hello,
I have been experiencing a strange problem the last couple of years.
The moment I start falling asleep, I wake up screaming, jerking awake.
Sometimes i wake up inhaling, as if i cant get any air. The doctors I
have talked to seem to never have heard of it. I take a number of
antidepressants, including mirtazapine. I would appreciate any comments.

You describe symptoms that are a classic sleep stage disorder. There is a risk of
other things going on that you may be unaware of, so it's important to get good data
about what you experience. Have you ever slept in a sleep lab, where they do
polysomnography? That's what you need, is a full assessment of your sleep
architecture.

There is a normal sequence of events that organize the shutting down of the brain
and body for sleep. You have some of those processes occurring out of sequence. The
significance of the disordering, and just what might be done about it, depends on
gathering precise information about exactly what is happening. Having frightful
experiences around sleep is not in your best interest, I'm sure you'd agree. Your
experience has a simple explanation, if that's any consolation. What you need is a
referral to a sleep disorders clinic for a full sleep work-up.

 It might be as LAR writes--that it's a sleep disorder--but I've had
this with mirtazapine, and a friend has, too. You know of course that
your skeletal muscles are paralyzed during dreams. You just lie there
and dream. I think what's going on is that the mirtazapine does
something to the dreams--making them more real? more likely to awaken
you? and so on--that one becomes more and more likely *consciously* to
experience this paralysis, almost invariably when waking *from a
dream.*

MY aforementioned friend and I developed a way to turn this from
something frightening (terrifying, really) into something not so
bad--to think of it as a game, labeling to ourselves the stages of
renewing consciousness: "Look, I can't move . . . look, I can wrinkle
my nose . . . " until the Holy Grail of awakeness and eukinisia was
achieved.

Bottom line: Uncomfortable side effect of mirtazapine; some puny ways
of dealing with it; others have had it.
