| Celebrex Renee S 05:58:56 |
| | Hi all,
I haven't been in here for a long time but I just thought I'd stop in to see what's being said about Celebrex. I take a 200mg capsule twice a day. Wondering what I will take if they pull this drug from the doctors. I have been without and I can't move well after five days of not having it. I am also taking Enbrel and Darvacet. I haven't talked with my Dr. yet. I have an appointment on the Jan 27.
Just wondering, Renee

| | Hi, I'm new to this site and am glad I ran across it. I have /had degenerative disk disease since 2003, just started hurting one day and had to have surgery 6 months later. Nothing worked to help the pain. I had a diskectomy, laminectomy with fusion of L4&5 with Bone Morphogenic Protein. After the surgery I did fairly well then started hurting really bad again. Doc thought I might had another bad disc but I didin't, the pain continued and worsened. He went in and did another surgery and removed the hardware and did another laminectomy. I am still hurting!!! He says he can't understand why but that there are a lot of things they still don't know in the medical field. I am now scheduled to go to a Pain Clinic next week.
I filed for my SSD in January of 2006, was denied in May, I have hired an attorney and am appealing. I am unable to sit or stand for any extended periods of time. We have been forced to file bankruptcy and now I have more medical bills to pay. What a shame it takes so long for people who need help!
I have been told my problem is hard to prove to get disability. Can anyone give me the average length of time it takes if you are approved? I hate going thru this. It took me a while to accept that I was going to have to file for it. Guess I should have done it sooner.

| | Internet Pharmacy Doctors Charged With Peddling Diet Drugs 08-02-2006 18:58 MARYCLAIRE DALE
PHILADELPHIA -- Two doctors helped peddle millions of dollars' worth of diet drugs through an online pharmacy, approving thousands of prescriptions without seeing anyone in person, according to charges filed Wednesday.
By signing off on the orders, Doctors Ranvir Ahlawat and Steven Klinman helped RxMedicalOne.com take in $33.6 million in nine months, according to the indictment.
Most of the orders were for highly addictive, controlled drugs, they said.
"This is high-tech drug dealing," said U.S. Attorney Patrick Meehan.
Ahlawat, 42, of Toms River, N.J., and Klinman, 57, of Elkins Park, Pa., reviewed the questionnaires customers sent in with their orders, prosecutors said. Ahlawat reviewed 1,000 to 1,500 a day at times.
Their lawyers did not immediately return calls Wednesday. A message left after-hours with RxMedicalOne.com also went unreturned.
Prosecutors said more people could be charged in the case. They said five doctors approved more than 350,000 prescriptions from September 2003 to May 2004, most for weight-loss drugs such as Phentermine and Adipex and sleep aids such as Ambien.
The indictment also charges pharmacy operator Michael Bezonsky; pharmacist Alexander Atchildiev, who is accused of shipping the drugs without ensuring the prescriptions were valid; and Thomas Beaulieu, who helped Bezonsky run the business.
"Fundamentally, the government's theory is that every prescription requires a face-to-face meeting with the equivalent of Dr. Welby, and that is out of step with the Internet world," said Bezonsky's lawyer, John M. Vandevelde.
A lawyer for Atchildiev did not immediately return a call. Beaulieu's lawyer, Benjamin Gluck, said his client was charged under a statute in which criminal intent is not required.

| Novel approach to the treatment of RA Ironjustice@Aol.Com 20:33:47 |
| | <<snip>> IL-6 is a candidate target for therapeutic inhibition <<snip>>
http://arthritis-research.com/content/8/S2/S4
Because of the central role played by IL-6 in a number of manifestations of these inflammatory diseases, IL-6 is a candidate target for therapeutic inhibition as a novel approach to the treatment of RA, sJIA and perhaps other related rheumatic diseases.
---------------------------------------------------------------------------В--------------------------
Induction of interleukin-6 by coal containing bioavailable iron is through both hydroxyl radical and ferryl species. Zhang Q, Huang X J Biosci. 2003 Feb ; 28(1): 95-100
Coal mining causes health problems, such as pneumoconiosis. We have previously shown that prevalence of pneumoconiosis in workers from various coalmine regions positively correlates with levels of bioavailable iron (BAI) in the coals from that region. In the present study, the nature of reactive oxygen species formed by BAI in the coals
and its mechanisms of the induction of biological responses were investigated. Human lung epithelial cell line, A549 cells, were used to
examine the induction of interleukin-6 (IL-6), a pro-inflammatory cytokine, which is known to play a crucial role in the development of pneumoconiosis. We found that levels of IL-6 protein as well as its mRNA were significantly increased in the cells treated for 24 h with 20
microg/cm2 of the BAI-containing Pennsylvania (PA) coal; for example we
observed 6.7-fold increase in IL-6 protein. Levels of IL-6 protein in cells treated with the Utah (UT) coal containing low-BAI were only 1.9-fold of the control levels. The enhancing effect on the IL-6 by the
PA coal was similar to that caused by hydrogen peroxide. Superoxide dismutase (SOD), catalase (CAT), and N-acetyl-L-cysteine (NAC) all had inhibitory effects on the PA coal-induced IL-6 formation. However, CAT had the least protective effect as compared to SOD and NAC. Our results
indicate that BAI in the PA coal may induce IL-6 through both ferryl species (via iron autoxidation) and hydroxyl radicals (via the Fenton/Haber Weiss reactions).

| Chikungunya to cure Arthritis? Gerhard Nowak 12:19:39 |
| | Hi there,
I didn't post yet on this forum, although I have Arthrosis in my knees since 8 years. I had to stop sports two years ago, because my knees started swelling from inside after training hours and it took approx. 2 weeks to diminish after.
Recently I got the Mosquito disease "Chikungunya" which is the same like "Ross River Fever" , a malaria kind of infection.
I had terrible pains in all my joints during the last 3 months and I felt like 10 years older.
Actually the point is, that now, after all the symptoms are gone, the swelling is gone as well!
I tried running on the beach - I am living on the Island of Mauritius - excessively, and ...... I couldn't believe it..... there was no swelling of my knees anymore! And I don't take tablets or else, never.
Is this only temporarily or did I find the real cure for the Arthritis?
Would like to have soime more opinions.

Experts Weigh In: Shortage of Patients In Cancer Trials Doctors Grapple With Lack of Volunteers for Cancer Studies
"We have a lot of trials where we can't get enough patients, the trial closes early, and that means agents (drugs) that might show a lot of promise in treating cancer never get to the market........" Dr. Julie Vose, chief of oncology/hematology at the University of Nebraska Medical Center
The American Cancer Society offers a computerized service that compares each patient's medical situation (such as type of cancer, how far it has spread, any treatments they have already had) and personal preferences, with the inclusion and exclusion criteria for thousands of cancer clinical trials." Dr. Ted Gansler, director of medical content, American Cancer Society
There are more than 400 cancer drugs now in clinical trials, many of which are experimental medicines with the potential to treat some of the most lethal and common forms of cancer.
The only problem is that researchers are running out of cancer patients to test all these new drugs on.
"Only about 3 percent of cancer patients participate in cancer clinical trials," said Dr. Robert Comis, a clinical trial expert and board president of the Coalition of Cancer Cooperative Groups.
Surprisingly, the most common reason patients don't enroll is that they don't know they can.
The Coalition of Cancer Cooperative Groups did a recent survey and found that only 10 percent of cancer survivors knew of any clinical trial for their specific cancer.........because only 10 percent of doctors tell patients about them.

| back to the doc again Becky 23:04:14 |
| | Hi all, I go back to the foot doctor again tomorrow, I have been trying to get some good custom orthodics made and it is taking forever, the first pair was so wide wouldn't fit in any shoe, so went back and he cut them a small amount the plastic hurts my arch sometime but tried to get another pair of shoes, since they discontinued the one I was wearing, anyway orthodic was still a little bit to big in heel so took back, well he made it narrow alright but now when I put my foot on it is almost all the way off the arch, so back again tomorrow and I am sure I will need a new pair and I don't think I should have to pay another 400 for them. My feet have been hurting which makes my back hurt, but lately I noticed that the toes fell like they are on fire and at night you feel pains in the base of both big toes, when RA is acting int he feet what exactly do you feel? When I was first diagnosed I have erosions in the 1 and 5 metatarsal so they were there, and then been on enbrle and mtx but the last foot x-rays should some more erosions, so I am not sure if hits is working, but I had only been on enbrel a month before those pics had been taken so it could have been from the years with just mtx. I am so discouraged, the feet hurt which makes the back bad, it was spasms last night and I was told that RA has nothing to do with the back hurting so I am at a loss. How do you know when your RA is progressing> I have an appt with the RA doc in 3 weeks so I will talk to him also, he may suggest Remicade, but I am not sure about hat one, since I have heard so ,many people say that they get allergic reactions. Well any infor would be great Thanks Becky

Title: The Efficacy of Glucosamine and Chondroitin Sulfate in Patients with Painful Knee Osteoarthritis (OA): The Glucosamine/chondroitin Arthritis Intervention Trial (GAIT) Category: 7. Osteoarthritis-clinical aspects Author(s): Daniel O. Clegg1, Domenic J. Reda2, Crystal L. Harris3, Marguerite A. Klein4, for the GAIT Investigators. 1University of Utah, Salt Lake City, UT; 2VACSP, Hines, IL; 3VACSP, Albuquerque, NM; 4NCCAM/NIH, Bethesda, MD Presentation Number: 622 Abstract: PURPOSE: Glucosamine (G) and chondroitin sulfate (CS) are widely promoted to "reduce joint pain and provide support for healthy cartilage and joint function." GAIT was designed to rigorously assess the efficacy and safety of these agents alone and in combination. G and CS were required to meet pharmaceutical standards as GAIT was conducted under an Investigational New Drug application. METHODS: Patients were =40 years of age with knee pain (WOMAC Pain 125-400 mm) of at least 6 months duration and x-ray evidence of knee OA [Kellgren-Lawrence (KL) Grades 2 or 3]. Patients were randomly assigned double-blind to placebo (P); G(Glucosamine HCl 500 mg) tid; Sodium CS 400 mg tid; G+CS at the above doses tid; or celecoxib (CE) 200 mg daily. All patients were allowed up to 4000 mg daily of acetaminophen (APAP) as rescue analgesia, except within 24 hours of study visits. Allocation was stratified by Center and by WOMAC Pain severity (125-300mm and 301-400mm). Patients were evaluated at baseline and weeks 4, 8, 16 and 24. The primary outcome measure was a 20% improvement from baseline in WOMAC Pain at week 24. Adverse events were documented at each visit. Analysis was based on intention-to-treat. RESULTS: 3238 patients were screened at 16 US academic rheumatology centers. 1583 were randomized and 1258 (80%) completed the study. Baseline characteristics were: mean age 58.6 years, BMI 31.7 kg/m2, OA symptoms 10 years, 64% female, summed mean WOMAC Pain 236В±73mm (206mm for 125-300mm stratum, 341mm for 301-400mm stratum), 59% KL Grade 2, and 78% were in the 125-300mm WOMAC Pain stratum and were evenly distributed across all arms. The response rate for CE (70.1%) was higher than the response rate for P (60.1%) in the primary outcome analysis of all patients (p=0.008). In the 301-400 mm WOMAC pain stratum, the response rate for G+CS (79.2%) was higher than P (54.3%) (p=0.002). Secondary outcomes in the 301-400 mm stratum, including 50% WOMAC Pain response, WOMAC Stiffness, WOMAC Function, HAQ, patient assessments, and use of rescue APAP all demonstrated changes consistent with the primary outcome. Adverse events were generally mild and evenly distributed among the groups. Response Rates by Treatment Group and Pain Stratum All patients WOMAC Pain 301-400mm WOMAC Pain 125-300mm P 60.1% 54.3% 61.7% CE 70.1%** 69.4%В¶ 70.3%* G 64.0% 65.7% 63.6% CS 65.4% 61.4% 66.5% G+CS 66.6%+ 79.2%# 62.9% ** p= 0.008 CE vs. P + p= 0.09 G+CS vs. P В¶p = 0.06 CE vs. P # p = 0.002 G+CS vs. P * p= 0.04 CE vs. P
CONCLUSIONS: Combination G+CS is effective in treating moderate to severe knee pain due to OA. The lack of response in patients with mild pain may be due to a floor effect, limiting ability to detect response. All study agents were well tolerated.

| glucosamine & chondroitin Califchief 22:14:00 |
| | RESULTS OF GLUCOSAMINE, CHONDROITIN NIH TRIALS ARE HAZY
The long-awaited National Institures of Health-funded research trial on the use of glucosamine & chondroitin for osteoarthrits have been completed. Conflicting media interpretations - some saying these dietary supplements work, and other saying they're a waste of money - leave consumers stranded between two extremes. So what to think? The six-month, double-blind placebo study evaluated glucosamine hydrochloride and chondroitin sulfate for treatment of OA-related knee pain. The 1,583 patients were classified as having either mild knee pain or moderate-to-severe knee pain. Most had mild knee pain.
Participants were divided in five groups, each taking one of the following: > Glucosamine hydrochhloride, 500mg three times daily. > Chondroitin sulfate, 400mg three times daily. > Glucosamine-chondroitin combination in above doses. > Celecoxib (Celebrex), 200mg daily. > Placebo pills.
To be included in the results, participants had to experience at least a 20% decrease in pain. The results are somewhat surprising. Those taking the placebo had a very high response rate (60.1%). All participants considered, glucosamine and chondroitin were no better than the placebo in reducing knee pain. Only celecoxib, the prescription non-steroidal anti-inflammatory (NSAID), worked better than the placebo, though just 10% better. There was good news, howver, in the smaller number of participants with moderate-to-severe knee pain: the glucosamie-chondroitin combination worked 25% better than the placebo for relieving more severe pain. Interestingly, chondroitin alone was found to reduce swelling in those who had swelling or fluid in the knee, while the glucosamine-chondroitin combination did not. The bottom line: glucosamine and chondroitin are deemed generally safe and might be worth trying for OA, with glucosamine sulfate as perhaps the first choice. Those with more severe pain may wish to use a glucosamine-chondroitin combination product. Keep in mind that it may take up to 2 months to see any benefits.

Osteoarthritis
This common degenerative joint disease is an expression of 'wear-and-tear'. Thus athletes or overweight people develop osteoarthritis in large joints (knees, shoulders, hips) due to loss or damage of cartilage. When cartilage is lost, the bone next to the joint is also eroded. There is some inflammation, but not as much as in other types of arthritis. Nevertheless, osteoarthritis is responsible for considerable pain and disability in older persons.
